Chattanooga Cancer Support Services Referral Process
Step One
Call our office or talk to your Oncology provider about making a referral to Chattanooga Cancer Support Services (CCSS).
Step Two
Your oncology office will fax a CCSS Referral Form, physician order for services, face sheet, any recent imaging, and last hospital or office note to 423-682-8892. If medications are being ordered, the referring office must notify CCSS immediately.
Step Three
A nurse navigator will call you to conduct a short screening to see if you qualify for our services.
